Regarding "Which of the following are true about .war files?" I think answer 4 is correct. Because EJB components can be part of either jar inside WEB-INF/lib or as class in WEB-INF/classes of web module deployed as .war file. Hence 2nd correct answer should be 4 not 1.

I suggest to re-phrase this question
You must place a web.xml descriptor file, if it is used in web application, into the WEB-INF folder inside the .war file.
Otherwise question is ambiguous.
